How did major fashion industries start out?

Before I go into the history of each designer company, I wanna know how big fashion brands and companies/industries develop, and im not talkin about macy’s or dillards but like marc by marc jacobs, Prada, D&G etc. how do they become so large and on top of the fashion world? They go to college and start out at a lower level (notice most designers are older)
